In order to receive adjustments for a specific learning disability, AD/HD, or any other special need, the student’s written documentation* must include:
- The nature and the severity of the student’s disability or need
- Specific prescription(s) for addressing the disability or need within the school setting (i.e., a list of recommended school adjustments for the student)
- Currency: within the last three years
The documentation provided will be used to develop a Learning Profile which serves as notification for teachers of a student’s diagnosis, strengths and weaknesses, and adjustment recommendations. The Learning Profile also is used as supporting documentation for modification requests for standardized tests and post-secondary schools. Documentation regarding special needs becomes part of a student’s cumulative record active file.
Parents will also be asked to sign a release of information, which authorizes the school to exchange information with designated agencies (i.e., doctors, public schools) as necessary.
